Accident and Injury Claim
To submit a claim under our accident policy, please complete this FORM.
Requesting Certificate of Insurance
Please review all steps prior to commencing your request for a certificate of insurance.

Step 1: Go to COI REQUEST FORM
Step 2: Fill out all required Club Information
Step 3: Click Next
Step 4: Enter the requested contact information and in the “CERTIFY TO” field (i.e. the facility operator, school board, city, town etc.)
Step 5: Click Next
Step 6: Add any Additionally Insured
Step 7: Submit (Can take up to 2 business days to receive insurance documents)
